pIntegrated Oncology Network ION  is committed to protecting the confidentiality of the information we maintain ION owns and provides administrative services to oncology physician practices ION is notifying patients whose information may have been involved in an email phishing incidentppOn May 9 2025 we concluded our investigation of an email phishing incident that resulted in unauthorized access to patient information in a small number of email and SharePoint accounts The investigation determined that unauthorized parties accessed a small number of email and SharePoint accounts between December 13 2024 and December 16 2024 Although the likely purpose of the unauthorized access was to perpetuate an email phishing scheme certain emails and SharePoint files were accessed by the unauthorized partiesppA review was performed of the emails attachments and SharePoint files that were or may have been accessed and it was determined that they contain information pertaining to certain patients The information potentially involved includes names in combination with one or more of the following addresses dates of birth financial account information diagnosis lab results medication treatment information health insurance and claims information provider names andor dates of treatment For a small number of individuals the incident could have involved their Social Security numbers To date there is no evidence that your specific information has been misusedppOn June 13 2025 ION provided notifications to oncology physician practices that have patients whose information may have been involved in the incident ppBeginning on June 27 2025 ION is mailing notice letters to patients whose information may have been involved in the incident If an individual believes their information was involved and have any questions about this incident please call 8553610308 Monday through Friday between 800 am 800 pm Central Time except for major US holidays ppFor individuals whose information was or may have been involved in the incident we recommend that they review the statements they receive from their healthcare providers and health insurance plans If they see any services that were not received they should contact the provider or health plan immediatelyppWe regret any inconvenience or concern this incident may cause and take this matter seriously To help prevent something like this from happening again we are providing additional cybersecurity training to our staffppHeadquarterspp104 Woodmont Blvd Suite 500Nashville TN 37205Phone 6154677400ppHeadquarterspp104 Woodmont Blvd Suite 500Nashville TN 37205Phone 6154677400p
